Got it - after a call to RIM, it turns out the Blackberry Synchronization Service had crashed (or did not start).

Once it was restarted, my Enterprise Activation appears to be working on my 4.0 devices.

Funny thing was we were still receiving emails on our 4.0 devices even though the EA process did not complete.
