4.5.0.81 is a pretty old OS. I know it's the last released Tmobile version, but going to another version may help. Other than that, maybe it's a hardware issue as the batteries seem to be showing ok in your wife's phone. Unless you can warranty exchange it, I'm not sure what you can do to fix that.
