I ended up talking to BlackBerry tech support via my Bell Mobility tech support. In my case, I have a BlackBerry Curve 9300 with the latest OS... 2921 / 666. I was directed to use the BB browser and typed in the following URL
cap E and C
and downloaded the latest BIS setup app and that fixed it. It looks like something got corrupted with the BIS setup app.
