There are a couple of options out there - iSkoot is in beta, it uses your phone connection to pass through calls to skype. Yes, it does use minutes, but it's a lot cheaper calling internationally than through your cell plan, and it solves the call quality on an edge connection problem.
Shape Services has a skype client too. I've never used it personally.
If you're looking to save on international calls, Packet8, a VOIP provider like Vonage has a blackberry client that intercepts when you dial international calls and passes them through your home phone account on Packet8 . Calls to canada, for example is free. The only reason I looked for a skype bb client are for the international calls, as I may be cancelling the home phone Packet8 account.
