Here are the steps.

Install Desktop Manager (looks like you've done this but make sure it's the latest version 4.2 SP1)
Download and install the OS package you downloaded
Delete the vendor.xml located in c:\program files\common files\research in motion\apploader. (if your 7290 is Cingular branded then you can skip this step but it doesn't hurt anything to delete this file.)

Plug in BB, open Desktop Manager. It should prompt that an upgrade is available. If it doesn't, just open Application Loader.

Uninstall ALL BlackBerry software (unless it's the latest version as noted below)
Download and install Desktop Manager 4.2 Service Pack 1 (https://www.blackberry.com/Downloads/entry.do?code=577BCC914F9E55D5E4E4F82F9F00E7D4)
Download and install the latest firmware for your device from your provider:

Cingular v4.1.0.351 (https://www.blackberry.com/Downloads/entry.do?code=577BCC914F9E55D5E4E4F82F9F00E7D4) (Package Version: 4.1.0.466, Software Platform: 1.8.0.149)
T-Mobile v4.1.0.309 (http://www.t-mobile.com/bbupgrade/) (T-Mobile phone number required)

Run Desktop Manager - it will automatically upgrade your device's firmware
