You have to remove the Blackberry related information from his exchange mailbox.
If you cannot access the old server:
Create a new user account on the new server,
wait a few minutes,
delete the user account, and check the Delete all blackberry information checkbox.
after that, you should be able to activate the user.
If this does not help:
erase the new device,
export the users mailbox to a pst file
delete the user mailbox
create a new mailbox
import the pst into the new mailbox
create an account on the new BES
activate the user.
