Default Upgrading to new device causes Software Config not show up?

We have been having issues with upgrading several users phones lately. The user gets a new device, we set the activation password in BES and they re-activate their new phone with the enterprise server and go on our way. We are pushing out a software config to put a shortcut on the users phone to go to a website. Lately this icon has not been showing up on the new device after activation. After much fiddling with 2 of the users we got the icon to show up again but not consistently.

Is there a "fool proof" way of upgrading a users phone to a new device without deleting their BES user and re-creating them?

We have BES v5.0.3.31 and the BB OS version is 7.0.0.374
