Default BlackBerry Enterprise SBE Activation problem

I'm having a lot of problem getting a Pearl 8100 BlackBerry device activated with the BlackBerry Enterprise server.
The device sits at Activating [email address] for a long time, until it comes with the message: "The server is not responding. Please contact your system administrator"
The BlackBerry Manager doesn't show any changes, the user remains on the status initialising.

I've tried about everything I could find on these forums, google and the RIM knowledge base but haven't had a single success so far.

Any suggestions to what I should do, or any ideas about what could be causing this problem?

Additional information:
Windows Server 2003 SP 1
Exchange 2003 Version: 6.5.7638.1
BlackBerry Enterprise SBE 4.1 SP 4
Connection to SQL server: ok
SRP key: valid
Network Connection (SRP): Successful
BES account is view-only administrator and has sufficient rights to send as, receive as and administer store on the Exchange mail store
Mail with dat file gets to the inbox and is remove almost instantly, I'm assuming for processing by the BES account
