My issue was similar. I couldn't get 2 way traffic with calendar entries. I was told by BB support that I needed the Exchange 2003 System Manager Tools installed on my BBES.
Turns out, when that was done i recieved many more problems with not being able to see the users list from Active Directory. But, i had the same issue in Outlook profiles creations. So, I opened Outlook to find that it said a program was recently installed that was conficting "would you like Outlook to fix it?"
So, when I clicked yes the BBES was working again. Further digging found that this was tied to a MAPI32.dll issue of sorts. I guess Outlook replaced the file or something.
Anyway, look into MAPI32.dll issues.
