Default VB6 Syncronization program for a CRM

I have a BlackBerry 8830 from Verizon and am developing a custom interface for VB6. I am using the sample code provided at na.blackberry.com/eng/developers/resources/journals/jul_2005/vb_desktop_addin_pt_1.jsp]BlackBerry - BlackBerry | Wireless Handheld Devices, Software & Services from Research In Motion (RIM). My trouble is the fields are not mapping properly.

I have found a few fields (see below). For example the number 2 should equate to last name but it is going to the phone and I can't even find the last name???

I also don't understand why I need to look at the SyncDemo sample? It uses it's own database and not the devices database.

Any ides? I am using the Desktop Software v4.2.
