Default HTML Mail Approval button

I want to put a button in a normal email. When you click the button I want the output to be an email reply with the subject line populated by job ref, decision made, and user verification. That all seems achievable with HTML and maybe a bit of javascript embedded in the HTML mail sent to the user.

However I also want to ensure the following:

1) The button will only work for the person to whom it was sent and could nto be forwarded to someone else for instance.

2) That once approved no further approvals are possible. I am playing with idea of deleting the mail when approval is made and sent but maybe there is another way.

How would you do this?
