Default Auto-run on startup

I know that this has been discussed earlier in this forum and in others. But I still have the problem of getting my application to auto-start. I have followed exactly the documentation and what has been suggested, signed the application but it still does not work.
It does work on the simulator, but not on the BB 8800 ( v4.2.1.74) that I am using. Also, I tried one of the application from the sample program and it still did not work.
Is there anything that I am missing? Or is there any setting on the device to enable this feature.

Any help will be appreciated.

Sorry if this advice is obvious for you, but for me I seemed to have to do a pretty "hard" reset to my device in order for the "auto-start" stuff to kick in. I think the only time I've seen the auto-start portion of my code triggered on the device is if I pulled the battery out and put it back in, or if other applications were updated through the device manager and for some reason this triggered a full hard reboot (I believe the Blackberry device software was updated).

I'm sure others will have a better perspective on this.
