Seems you are looking for a service from network provider. T-Mobile has such a service where you pick a tune that the caller hears when they dial your number. T-Mobile calls it "CallerTunes" and charges $1.49 per month.

My wife has it. I don't think it's monthly, just the $1.49 price tag to download it.

Hey Blake... the TMO charge for Caller Tunes is monthly $1.49...

I know because my son put caller tunes on his account (in my master account). Now I get to hear "She thinks ma tractas sexxxxy..." every time I call him. And I get the privilege of paying for it.
