SIM Card subsidy locks don't show up as SIM security. You need to go Options->Advanced->SIM Card. When in the menu, type "MEPPD" and a new section of menu will appear. If any of the items under Personalisation are "Enabled," that means it's locked. Typically, it's the Network that is enabled. T-Mobile can give you the unlock code.
As far as erasing the device, you can follow the directions in AfterWord's post. But, that won't remove the IT policy that has been installed. There's a sticky post
