Hmmm...

Work related (coworkers, boss, etc) - Aflac Three Blind Mice (From AFLAC's website)
Wife - a squeaking dogs toy
friend - Murgorsky Night
friend - Coke theme (he works for Coke)
Sister - "House" theme (she's a nurse)
uncatagorized calls - Bub_Rinehart (Harley Davidson starting and riding away)

I've also used Hello Moto (on a Blackberry, yes), stock iPhone ringer, old Nokia ringer, and the "Gooooooooooood Morning Vietnam.... hey this is not a test this is rock and roll...." scene from the same movie.

If I need a loud ringtone, I have a couple of airhorn sound from Wolo's website that I ported over. I have a Star Trek air horn, Macarena AH, and the obligatory Dixie.
