If I run the Backup and Restore from the Blackberry Desktop Manager, what exactly gets backed up? I would like for all my data to be backed up, including any applications, themes, etc., I might have downloaded and installed from the Internet.

when you start the backup it will give you choices.
Most of the times it is data. But it also images your apps so yes you will get the apps back when you do a restore. If you are trying to shift over to a new BB then switch device is a better way of doing it.
AS REGARDS THE THEMES I think that they dont get backed up. Would suggest you wait for a few more responses on this topic.
You can use the work around of the switch device wizard which is posted in one of the threads here to ensure you have got all your third party apps backed up.

any1 knows if on the backup, the contats are keept as xls(as in a palm) ??

No. The backup file (everything is in one file) is in an "IPD" format. There are some IPD format readers out there. But it is NOT stored as XLS.
