At this point you need to narrow it down. Could be device hardware or software, cable, or computer. Try to eliminate each one. Try different computer, different USB ports, different cable, hard reset the device...
It sounds like maybe the device is disconnecting or being disconnected and connecting again. Does the computer indicate this is happening?
