Default BB Calendar items not syncing to Outlook BIS

Environment: BB Curve 8703, Windows XP with current SPs and updates, Windows Server 2003, Exchange Server 2003.

User was having problems with GPS, called Verizon tech support, installed DM 4.1. Afterward when he synced, all calendar items from that date forward disappeared in his Outlook Calendar, but they were still on his BB. User erroneously chose BES instead of BIS. I have done a repair on the installation, installed 4.3, reset the sync configuration settings, tried just syncing BB appointments to Outlook Calendar, but they're not coming over to Outlook. I checked Exchange for undeleting Outlook items, but the missing items are not there either. Current new items on BB and in Outlook are syncing correctly.

Uninstalled DM and installed DM 4.5. Calendar items on BB from 8/6 forward are still not appearing. Noted that in BB calendars both CICAL and CMIME exist for user's email address. Not sure which contains correct calendar data.

Goal: transfer calendar items on BB from 8/6 forward to Outlook.
