Sorry to say, but this is one of the hardest problems to fix. Here's some things to try.
You shouldn't need any special drivers, but make sure if there's anything special the machine's manufacturer requires for the USB ports, you have the LATEST version of their drivers handy. If you do need special drivers, and the ones you don't have are older, update them. Don't forget this may also mean Power Management software.
Speaking of which, make sure that if there's any power management of the USB ports, disable it (very rare though).
Try using MSCONFIG to disable as much as possible from startup including any services that don't look like their part of the OS. If that fixes the problem, add a few things back at a time until the USB stops working, and then figure out which one is the culprit.
If you have any software that addresses the USB ports - especially some kind of camera or disk drive software.
Go into Device Manager. From Device Manager's menu, select
View --> Show Hidden Devices, and uninstall everything under
Universal Serial Bus Controllers, then reboot. This will get rid of any 'phantom' USB devices. Reboot, and all the real USB devices should come back.
