Open Intellisync, click configure PIM, select the address book, click browse.
From here, there are two things that may work.
1) in the bottom section, highlight the contacts folder location, and click remove. In the top section, navigate to the contacts folder again and click add. If this prompts you to include subfolders, then you are golden. If not, check option 2.
2) On the top navigate to each sub folder highlight them and click the add button. They should then all be listed in the bottom, with the main contacts folder in bold. This will be the default location that new contacts (created on the BlackBerry) will be sync'd to in Outlook.
Both of these options will put all contacts into the same address book, so you will have to manually create groups if you want them grouped.
