Conflicts in Outlook's Contacts can cause sync problems, and are hard to find.
Change your Contacts view to a list view (I think Phone List is set up by Default) - make sure you have an Icon column. Then look for an icon with two yellow crossed swords. Open the contact and resolve the conflict or delete and recreate the contact.
Also, it's easy to accidentally drag non Contact items into the Contacts folder. Make sure all your Contacts are actually Contact items.
If all else fails run SCANOST.EXE (if you're connected to an Exchange server) or SCANPST.EXE (if you're not). With Outlook 2003, the files are under the C:\Program Files\System\MSMAPI\1033
