#1 was exactly the problem. I called to check on my ticket just a few minutes before your post and the tech noticed that I was setup for 3G instead of EVDO. We reprovisioned the phone and it took a couple of hours before my phone functioned as a modem but I'm running it now succesfully. Speedtests on speakeasy are showing ~1.5Mb down and 150-250Kb up. Now too bad. Thanks for the help.
2. Yes, even when this didn't work, CM could probe the modem just fine.
Now to figure out how to get PAM working under linux so I don't have to fire up vmware...
