Not sure I can help, but one thing to check is to make sure the users ACLs have all the appropriate permissions. Maybe some of the Domino servers in you cluster are not meeting the ACL requirements?...

I had the issue all the time on the T-mobile release, and a few times on the o2 release.

I went back to the Rogers release and have not had this issue.
