Well it appears it was fixed by just moving me to a different blackberry server (using the option to do so on the BAS). Apparently there is a corrupt agent of some sort - don't know the exact one -...

This is fixed. Turned out he was using a 2.5mm to 3.5mm converter with a 2.5mm headset. Once he used a proper 3.5mm headset it began working.

No, it must have already been set up for them (it was a replacement BB). The icon and email account appeared after activation automatically.
