It *should* activate when you do the device switch, but calling Verizon and having it done that way is much more certain. I'd definitely give that a try first.

Sounds like the BIS account in question is pointing at the wrong phone. Try logging into your Verizon BIS page and re-entering your new PIN/IMEI for this device. You can resend service books from...
