This happened to me on occassion - in the text screen the top right corner has an "EN" for English - oddly enough, you can tap it to change it - and I've done that a few times - I have an issue...

I just realized I had the red SOS symbol, I accessed my Mobile Network options and it states T-Mobile (instead of AT&T).

Go to the home page - click on the top of the screen then scroll down and access Options -> Accessibility and change the Home Grid Layout to "Default". I think you have it set to "1 column".
