It will directly unlock if a password is required. If a password is not required you have to use the * phone method. It's been like that since the Pearl came out with 4.2 a year ago.

Yeah, that's been a bug in the 4.2.1 OS's since the beginning. As you said, it's more of an annoyance than anything else. It doesn't affect anything.

It depends on the theme you're using. If the theme is a screen of icons then simply highlight the icon to move, hit alt + trackwheel, then select "move icon."

The order of the 5 icons on the home screen is simply the order of the icons on the other home screen. Reorder the first 5 in that list and you'll change your 5 on the home screen.

That is in Phone >Options >General Options. However, I believe that just is the volume that the phone automatically answers in. I don't think it affects how loud you can turn it up when on a call.

I would update the DM to the latest released by RIM. I would update to the 4.2.1.101 that was just released by Proximus, too, but that is up to you guys.

Just try deleting the vendor.xml file...

...all the more reason to believe this release from TMo is nothing new. It's the same as every other 4.2.1.91, to the extent that they didn't even change the vendor.xml file.

You shouldn't have to for TMo users but have you tried deleting the vendor.xml file? It's not unheard of to have an error in that preventing it from loading the OS.
