I've tried both the Rogers and Cingular upgrades on my 7100t and still have the low volume BT issue. I went back to T-Mobile's 3.8.

I tried using the latest Cingular and Rogers 4.0 upgrades, and still had the same issue as with T-Mobile's 4.0. I'm back to T-Mobile's 3.8.

T-Mobile BWC - 100mb upgrade

They let me upgrade to 100mb with no extra charge. According to them, their server may "downgrade" me if I don't use a certain amount of space, but, knock on wood, this hasn't happened yet.
