So I noticed that when my phone is in sleep/lock mode and I receive a call, it does not show the contact information. It only shows the phone number. With this, it doesn't even ring the custom tone I set for the person who is calling.

What to do to fix this?

Sounds like you have your address book protected.

Go to Options > Security Options > General Settings. Set Include Contacts to No

This is a BB security feature. There is no fix. When your BB is locked its for security reasons to now show the name of the caller to protect their identity just in case someone steals your phone.
