Desktop Manager appears to hate Cached Exchange mode for Outlook so in order for our contacts sync to work, we have to run all our PCs in online / offline mode. This is a real pain as it puts extra load on Exchange Server amongst other things.

I might be missing something here, but there’s no issue with cached vs. online modes, except that your client will need more bandwidth to maintain the connection. I’ve found it’s been much easier to use, especially if you’re on a network link with limited bandwidth (unfortunately, I’ve had to work with situations where I needed online (not cached) connections and could not get a good network link.
For the multiple contacts folders, are you adding them in the address book advanced options menu? What issues are you seeing that appear to be made worse by cache mode? For the non-public contact, have your users enabled off-line access within their mailboxes (within the mail setup on the Outlook client)?
