In the profiles on my phone I note that there is a "phone only" setting. I can not figure out what this setting accomplishes? Looked in the manual and tried to search this forum but could not find the answer.

Only phone calls will cause the BlackBerry to make an audible or vibratory alert.
Every other event (email, sms, calendar, IM, etc...) will be silent.

The alarm clock will also make an audible sound when on "Phone Only."

True... but the alarm clock is not an event that is managed within Profiles.
It's a separate module.

Means "Only Phone"...figured I'd be the wise guy

...though really the profile names are just for your reference. Out of the box Phone Only is phone only but you can completely customize it.
