How do I remove icon that install application?
 

Those icon leads to Blackberry virtual preload agreement and then the installation page.

However, once installed, the icon remains.

How do I get rid of it?

Move them into a folder and then hide the folder

You can't remove it permanently. Your provider will just push it back out to your phone if you do delete it.

The only solution is, as dankarlinski said, put it in a folder and hide the folder...

I did just that, move them then hide them.
