Few things to ponder upon -
1. UserOTAFM=0 - Are you sure that activation was successful for both users.
2. it looks like BES server is getting the email from Exchange fine. But could not deliver it to the device via dispatcher. Is dispatcher service up and running.
3. MAPICDO process generates dbg file after every email delivery. Not normal.

here is what I will do -
Make sure all BB services are running.
Make sure SRP is up and running. (BBSRPTEST is a good tool for that)
Delete the user from BES with an option to delete the information from mail server.
Wipe the device.
Activate it again. (if you have BES plan try EA). Observe the activation process.

All should happily work. If not I would uninstall BES, delete BESMgmt DB, uninstall MAPICDO. and do it all over once again.
