Duplicate Calendar items when meeting request changes

Hello, we have BES 4.0.16 in an exchange 2003 sp2 environment, users are calling about calendar items that are duplicated when they accept a meeting request update. the original one stays and a new one is created.
It tested with a Blackberry user and a non-blackberry user. It only seems to occur with the Blackberry users.
I tried wiping the device and reactivating it, and I also checked the mapi32.dll and cdo.dll version match on the BES and Exchange servers. What should i look at next? thanks.
