Has anybody had any major issues with this particular service pack?

I assume you can go straight to SP 4 without having to install earlier SP?

No issues with upgrade but, watch out for the MDS password through NOTESID resetting itself. Just the same as 2.2.2. You will need to reembed the internet password back into the file, also keep checking your server document. Essentially, follow the pre reqs as per a straight install and you will be fine.

No problems here, I went straight from 4.0 to 4.0 SP4.

Edit: Forgot to mention that the Domino version is 6.5.4 SP2.
