Default Blackberry devices being reactivated

We are having an issue where several devices are being activated wirelessly. It's not isolated to one certain device, carrier or firmware version as we see it so far.

1. Has anyone experienced this?
1a. if so do you know if data charges are incurred because of this?
1b. what is the cause?
1c. is this normal?

2. Is there a way to show when a device has been activated/reactivated? I could only find when the user account was created.

What do you mean they are being activated wirelessly....are you not aware it's happening and users are just calling in and saying their devices are going thru the activation?

There is a way to show when a device has been activated, but need to run a sql query against the database. You can use an itty bitty program call QueryExpress and run a query like this:
