They're mostly used for group calendaring in a BES (BlackBerry Enterprise Server) environment. If you have an appointment that you've set as Free, you will be shown as available when someone tries to set up a meeting with you at that time. If you've set that time as Busy they will see that you're not available (and hopefully try to schedule a time you are available - but not always!). Your status is always available to others when they try to schedule something regardless of the privacy settings.
I don't know if the BlackBerry Calendar itself is smart enough to tell you if you're entering an appointment that conflicts with another if its set to Busy - and not warn you if it's set to Free (in a non BES environment).
