This sounds most logical. Your issue looks like the carrier mis-provisioned your account. Possibly they were trying to change somebody else's account and borked yours. When you call they have a tendency to say, "Yeah yeah, we checked everything and the problem must be on your end." but they don't look closely and really only want to get you off the phone. Laziness and ineptitude are everywhere. You have to be persistent and make several calls sometimes for the same issue.
