Can the Blackberry browser be used via a squid proxy running NTLM authentication?

Not sure...

o Squid Proxy using non-NTLM authentication - yes

o Internal websites using NTLM authentication - yes

o Squid Proxy using NTLM authentication - never tried it but I'd have thought so.

This is what a Vodafone trainer appeared to indicate to me, but while I get the authentication dialogue, I am prompted over and over and finally informed I am using an unsupported authentication scheme.

What about 3rd party browsers like Opera Mini, do they use the WAP connection or the "other" one (what is the proper name for that?)?

The fix for the "Unsupported authentication scheme" problem is here:

Thanks, I checked but "Support HTTP Authentication" is already set to true and we have had several full service restarts recently :(

Is there anything of interest in the BES MDAT, squid or website that you're trying to access logfiles?

Verbose HTTP logging isn't turned on on the BES by default - you may need to enable it. Also (not sure if it's relevant to you) but if you're trying to get MDS connection (on behalf of a user Blackberry) to access an untrusted HTTPS server you'll need to persuade it to trust it.
