BES version advice.

Hi,
I am currently running BES for Exchange 4.0.2.8 and have a couple questions about BES version. I have not upgraded my software so far, because I was not having any problems and there are so many SP2/upgrade horror stories. I wanted to make sure I planned well.
My questions are:
1) Is there a universally agreed on version that admins should be running on? IE ... anything less then this is asking for disaster? Or, is the old rule of upgrade only when you need it true?
2) With all this NTP and BB talk about a patch for this so called USA mode going around. How does that play into this? Do I need to be on a specific version now, just in case I need to use this patch in a pinch?
3) Client software. If I am upgrade to SPx, do I have to have matching client software? Are there catches to that etc?
4) I am running my BES on a Windows 2003 SP1 server, in a 2000 domain (Exchange 2000 - Personal SQL). Any prep work involved on any of these pieces prior to a BES SP?
