You have a simple forward to blackberry email address.
You have access to your server SMTP logs, right?
What wireless carrier your users are on?
These copies of email messages, are they in webclient, too- or only on blackberry? How many copies of emails can you find in webclient?
These emails, do they have many recipients including distribution lists, or they are sent just to one recipient?
Can blackberry user reply to duplicate email from the blackberry, can they do "reply all"- and are there any issues with replies?
Did you check SMTP logs and can guarantee that your server sent email only once and didn't retry multiple times? If it did, what error message it received from the RIM mail server?
Can you post fresh SMTP logs for such 'bad' email? You can mask recipient name but keep message ID.
