I've had lots of mixed suggestions as to what should be on the main screen, mostly Friends Timeline vs. Public Timeline. I agree with you: I think it should be the Friends Timeline.
Auto-refreshing after an update could be a bit too frequent, esp. for more active Twitter users. However, I definitely plan to implement an auto-refresh.
The "new message count/indicator" would be nice, but yeah, finding the relevant API would be the trick. Should be able to look through the docs, or at least decompile whatever apps can do it.
Sound notification ... while it sounds like a good idea, it's likely to be the most annoying feature, too. I'll add that to the wishlist, though.
